If you are an Authorized Individual acting for the Account Owner under a Power of Attorney, you must provide a copy of the written Power of Attorney naming you as an attorney in fact, granting you the power to act on behalf of the Account Owner, and signed by the Account Owner or at the Account Owner's direction and notarized. If any documentation required to be submitted according to these enrollment materials requires two or more signatories, then please download, print and complete an enrollment form and mail it to the Plan accompanied with the relevant documentation. After this account is created online, the account will be under a 'pending approval' status until all the required paperwork has been received, reviewed, and approved.
